How to rollover your previous University Account to Fidelity or TIAA CREF

Fill out and submit your Salary reduction agreement (403b or 457b) or ARP Vendor Change Form (ARP).

*Make sure you submit this form to HR asap so that no more contributions go into your old account and you end up having to do this process twice.

       Step 2: Request the rollover paperwork from your previous account provider

ARP, 403b & 457b Accounts:
Call – 1-800-262-3862

Ask them for the “Contract Exchange” (403b) or “Transfer” (457 & ARP) paperwork.

They may ask you if you’re still employed and you’ll say yes that you are transferring to a like plan with another provider.

They can/will email, fax, or mail the form to you.

457b Accounts:
Call – 1-877-644-6457

Ask them for the “Transfer” paperwork.

They may ask you if you’re still employed and you’ll say yes that you are transferring to a like plan with another provider.

They can/will mail the form to you.

ARP, 403b & 457b Accounts:
Call – 1-800-428-2542

Ask them for the “Contract Exchange” (403b) or “Transfer” (457 & ARP)

They may ask you if you’re still employed and you’ll say yes that you are transferring to a like plan with another provider.

They can/will email, fax, or mail the form to you.

403b & 457b Accounts:

Ask them for the “Contract Exchange” (403b) or “Transfer” (457) paperwork.

ARP Accounts:

Ask them for the “In-Service Transfer” form.

They may ask you if you’re still employed, you’ll say yes that you are transferring to a like plan with another provider, and that you’ve already submitted you Vendor Change Form.

Once you receive the paperwork then you need to fill it out. You should know most of the information to complete this form except the information of your new provider which you may gather below:

Fidelity:
Make the check payable to “Fidelity FBO Your Full Name
Mail to: Fidelity Investments, PO Box 770002, Cincinnati, OH, 45277-0090
Account number: Fidelity just uses your social security number as your account number.

 

TIAA-CREF:
Make the check payable to “TIAA-CREF FBO Your Full Name
Mail to: TIAA-CREF, PO Box 1260, Charlotte, NC 28201-1260
Account number: You may put your social security number or you can log into your account or call TIAA-CREF to get your actual account number: 1-800-842-2252.

Next you will need to submit your transfer form back to us so we can make sure it is completed correctly and can forward to the applicable parties to finalize your transfer:

Upload it into the “Shared Documents” folder in your rFPW.

Attach and Email it.

Monitor for any phone call or letter from the old provider that indicates that they need additional information or that they have rejected your transfer. We will let you know as soon as TIAA-CREF or Fidelity receives the transfer, which will mark the successful conclusion of the process. You will then be able to see your funds in your new account on your rFPW – Login Here.
